I think he does, because on page 216-217 in my book it says that Smaug thrust his head at the opening and pursued Bilbo with flames from his nostrils.

Well I haven't read the entire book but I have read chapter 12. I'm guessing it's just part of the story, because in most cases good prevails over all and it was just to keep the story going.
